“There are a lot of reasons that our pediatric patients need to start becoming more independent and learn to take care of themselves as adults,” explained Erika Jewell, the transition navigator for Miller Children’s and Long Beach Memorial Medical Center.… “The grant allows us to make our transition a more formal process,” said Jewell, who worked as a social worker at Miller Children’s for a decade before taking on a new position just more than a year ago as the hospital’s first transition navigator.
